Ontario Health atHome coordinates in-home and community-based care for thousands of patients across the province every day. They assess patient care needs and deliver in-home and community-based services to support patients' health and well-being. Additionally, they provide access and referrals to other community services and manage Ontario’s long-term care home placement process. Ontario Health atHome collaborates with primary care providers, hospitals, Ontario Health Teams, and many other health system partners to support high-quality, integrated care planning and delivery.

Ontario Health at Home is seeking a Chief Executive Officer (CEO) to lead the organization in the management and delivery of home care and long-term care home placement, while supporting co-design and modernization of key functions in collaboration with system partners, including patients and families.  

With a mandate to build a patient-centred, accessible and outcome-focused organization that is integrated and fiscally responsible, the CEO will enable a culture of high-quality, patient-focused care. Demonstrating systems thinking, continuous innovation, strong fiscal management and accountability for results will be critical. The CEO will develop and maintain a diverse and equitable organization, reflecting the broad ethnic and cultural complexion of the population including patients, families and staff.

This position provides overall leadership for the recommendation, development and implementation of a comprehensive strategy to ensure the continued delivery of high-quality home care services, while maintaining exceptional patient experience. It ensures organization’s strategies, plans and practices are aligned with the Ministry of Health’s (MOH) transformation plan. The CEO will be expected to maintain strong communication, partnership and alignment between Ontario Health atHome and OH (its parent organization), as well as MOH and the Ministry of Long-Term Care (MLTC). As Ontario’s health system continues to modernize and transform, the CEO will lead Ontario Health atHome in achieving its Board-approved strategic plans, while providing a stable platform for the delivery of care, ensuring continuity of patient care and workforce capacity to deliver on this important work.   

Reporting to the Ontario Health atHome Board, the CEO will provide visionary leadership to a complex and evolving organization. The successful candidate will champion innovation, fiscal accountability, and system integration while enabling an accessible, outcomes-focused care model. The CEO will foster a culture grounded in equity, diversity and inclusion, ensuring that Ontario Health atHome reflects the broad ethnic and cultural diversity of the communities it serves.

Key Responsibilities
Leadership

  • Provide executive leadership for daily operations, ensuring alignment with legislation, government direction, and Board-approved plans.

  • Ensure high-quality, patient-focused home care, long-term care placement, and related services.

  • Maintain oversight of operations and key relationships, especially with Ontario Health.

  • Serve as the public face of the organization and lead staff across care, planning, performance, resources, and communications.

Community and Stakeholder Engagement​​​​​

  • Engage transparently with patients, caregivers, providers, Indigenous and Francophone partners, and community leaders.

  • Collaborate with Ontario Health, MOH, MLTC, and local providers to build trusted partnerships and effective service models.

Governance, Legislation and Ministry Accountability​​​​​​

  • Ensure compliance with legislative and policy frameworks; advise the Board on risks and obligations.

  • Support Board effectiveness and meet performance reporting requirements with government partners.

Financial and Performance Accountability

  • Ensure responsible use of public funds and adherence to approved budgets.

  • Strengthen financial controls, staff performance, and government reporting.

Strategic Planning​​​​​​

  • Lead strategic and operational planning aligned with provincial health priorities.

  • Champion innovation, system integration, and improved patient outcomes.

Transformation

  • Drive the modernization of home care and transition to integrated care models.

  • Lead change management efforts, engaging staff and partners in cultural and operational shifts.

  • Anticipate evolving service needs, set clear performance goals, and foster innovation for system sustainability.

Ideal Candidate Profile
The ideal CEO is a strategic leader with senior executive experience in community-based care sectors. They possess deep knowledge of Ontario’s healthcare system, a commitment to patient-centred care, and proven success in system transformation and integrated care. Experience with Ontario Health, MOH, or MLTC is preferred, along with direct patient care experience.

They are politically savvy, collaborative, and known for integrity, empathy, and inclusivity. With a strong equity lens, they design culturally responsive services and engage patients in care. As CEO, they will be a trusted government partner and lead Ontario Health atHome in delivering on strategic goals and improving patient outcomes.

Requirements
The new CEO should possess the following:​​​​​

  • A master’s degree required in health, public or business administration.

  • A minimum of 15 years of progressive professional and leadership experience (or equivalent) in the health sector (governmental, non-governmental, not-for-profit, commercial), with several years of experience in a senior executive leadership role.

  • Demonstrated experience creating an environment that develops, unifies and integrates cultures to support innovation, accountability and a patient focus, while implementing progressive change management strategies.

  • Extensive experience working with and supporting a Board or governing body is required.

  • Strong transformational leadership skills and the ability to drive timely and significant change in a complex, geographically dispersed organization.

  • Understanding of the framework for health care planning and delivery in Ontario.

  • Extensive understanding of government, policy and programming.

  • Knowledge of current government and Ontario health sector systems and workforce trends within this sector.

  • Exceptional ability to lead and mentor senior leaders and inspire, coach, evaluate, share knowledge and engage staff and stakeholders.

  • Demonstrable experience in strategic planning and high-level delivery, translating organizational vision into successful outcomes.
